Privacy Policy

 

The personal information collected by CFT Psychology and Consulting (ABN: 49673587178) is also detailed in the online intake form. Personal information obtained includes your name, email, contact phone number, address, payment and billing information for processing cancellation fees, or to process service payments.

Collection of information and confidentility:

Consultations are conducted with professional confidentiality that is in accordance with the Code of Ethics of the Australian Psychological Society 2010, Health Records Act 2001 (Vic), the Privacy Act 1988 (Cth) and the Federal Circuit and Family Court Act (2021).

Information will remain confidential, except when:

1. Failure to disclose the information would place you or another person at risk
2. Your prior approval has been obtained to report to another professional (e.g., GP)
3. It is subpoenaed by a court.
4. For Federal Circuit and Family Court of Australia (FCFCOA) matters, assessments are conducted, and information obtained, in accordance with the Federal Circuit and Family Court of Australia (Family Law) Rules 2021. A separate consent form is available in relation to attending an assessment for a FCFCOA report.

Use and Disclosure

Personal information is collected and used for the purpose it was collected for (psychological assessment and intervention). Payment information is also used to process zoom online bookings (not for EAP services) and, from time to time, to enforce late cancellation or no show fees in the cancellation policy.

For telehealth, sessions will be conducted via a secure encrypted online platform called Zoom Professional. The session will be conducted ina private room with a meeting link that is personalized to you.

Storage of your health records

Electronic and paper records are kept in secure locked storage. All personal information collected during the provision of the psychological services will remain confidential and secure. Appointment information is kept in a diary management service called Halaxy however notes are not stored online.

Cancellation Fees:

For therapy sessions, a cancellation fee of $240 is required if the cancellation occurs 48 hours before your appointment.  Please be mindful of giving me as much notice as possible regarding the cancellation of a scheduled session.
